All applicants are required to fill out Application Form I. First time publishers and publishers applying for a block of ISBNs are required to fill out Application Forms I and II. Completed forms can be faxed or delivered to Technical Services Division, the National Library, St. Vincent Street, Port of Spain. Applications would only be processed after fees are received. Application fees are payable at the Accounts Department, NALIS, the National Library, St. Vincent Street, Port of Spain. Forms can be downloaded from the links below.
