12.1 How to obtain a Food Badge

Food Badges are issued by the Public Health Department, City Hall, 33, Frederick Street, Port-of-Spain. The days for business are Mondays and Thursdays from 8:30 a.m.

Port-of-Spain

To obtain a Food Badge for preparing food on premises located in Port-of-Spain, the applicant is required to submit to the Public Health Department, City Hall, the following:

(1) A medical certificate

(2) Two passport size identification photographs

(3) Identification Card, Driver's Permit or Passport.

Please note that the Form to obtain the medical certificate from the doctor is obtainable at the Public Health Department, City Hall, Port-of-Spain.

On submission of the above particulars, the applicant will be interviewed by the Public Health Inspector. Should the applicant be successful at the interview, the premises on which the food will be prepared is then inspected. Provided the place of food preparation meets the minimum public health requirements, it will then be registered. Following a lecture, the applicant receives the food badge.

Areas outside Port-of-Spain

With regard to food that will be prepared on premises outside the City of Port-of-Spain, the applicant is required to inform the relevant Public Health Department in the district where the food will be prepared.

The Public Health Inspector from that department will inspect the premises. Provided that the minimum public health requirements are met, the premises will then be registered. Following this, the applicant will be issued a document pertaining to the registration of food premises.

The applicant must then submit this document, a medical certificate, two identification size photographs, an identification card, Driver's Permit or Passport to the Public Health Department, City Hall in order to obtain a Food Badge.
