13.1 How do I obtain an Identification Card?

To obtain a National Identification Card an applicant must be -

a national of Trinidad and Tobago 15 years and over;

a Commonwealth citizen 15 years and over;

a Non-Commonwealth citizen 15 years and over.

Procedure

Go to the Registration Office in your area with documents such as -

A Birth Certificate and copy (the Birth Certificate must come from the Registrar General's Department).

Accompanying Affidavit and copy ( if name does not appear or correspond with that on the Birth Certificate).

A valid Passport and copy of the first four (4) pages (in lieu of Birth Certificate).

A Marriage Certificate and copy (if a married woman).

Appropriate documents from the Immigration Department (in the case of a non-national).

The applicant is required to fill out a form giving the true and correct information. He/she is then requested to either proceed to the photography room where a photograph will be taken or submit two (2) recent photographs either coloured or black and white size 1 inch x 1¼ inches.

A field investigation is carried out by the Registration Officer to ensure that all the information filled out on the form is correct.

Once the information has been verified, the Identification Card would be processed.

13.2 How do I renew my Identification Card?

You should proceed to the nearest Registration Office in your area taking with you the expired Identification card and make a request for a renewal. If married, walk with a marriage certificate and copy. At this point you may either have your photograph taken or submit two recent photographs.

When the card is ready, you will be called in. You will hand over your old Identification card and a new one will be presented to you.

13.3 What should I do if my Identification card is lost, stolen, damaged by fire or mutilated?

PROCEDURE

1. Go to the Registration Office in your area, state the reason and request Form 71 and a voucher form.

2. Make a report at the Police Station in your area where Form 71 will be stamped.

3. Proceed to the Commissioner of Affidavits who will sign (Form 71). You are required to pay $2.50.

4. Submit voucher form at the nearest Revenue/Warden's Office where you will have to pay $5.00 for the replacement. You will receive a receipt.

5. You will return to the Registration Office with the receipt and Form 71 where the Registration Officer/Clerk will take the necessary information.

6. A replacement Identification card will be processed to the registrant.
